Lesi\u00f3n en fase 1 (aguda inflamatoria). C\u00f3mo calcular el gasto energ\u00e9tico total (GET) y los macronutrientes.<\/p>\n<p><span style=\"text-decoration: underline;\">Os presento el caso<\/span>: Carlos es un futbolista semiprofesional de 25 a\u00f1os que mide 180cm y pesa 77kg (con un % graso de 9). Se ha roto recientemente en ligamentro cruzado anterior LCA) y ahora esta en su etapa de recuperaci\u00f3n de 9-12 meses (en la fase 1: aguda inflamatoria que son los primeros 5 d\u00edas post operaci\u00f3n).<\/p>\n<p><strong>Si en esta fase Carlos no cuida su alimentaci\u00f3n<\/strong> \u00abcomo cuando no estaba lesionado y jugaba\u00bb, lo m\u00e1s normal es que:<\/p>\n<p>&#8211; <strong>Gane grasa<\/strong> porque tiene menos actividad f\u00edsica y, por lo tanto, tiene menos gasto energ\u00e9tico.<\/p>\n<p>&#8211; <strong>Pierda m\u00fasculo<\/strong> porque baja la ingesta de prote\u00ednas porque no entrena como antes.<\/p>\n<p>&#8211; <strong>Retrase la cicatrizaci\u00f3n del ligamento<\/strong> por esa disminuci\u00f3n de la ingesta proteica.<\/p>\n<p>De esta forma, <strong>el resultado<\/strong> va a ser que <strong>vuelva m\u00e1s tarde y, adem\u00e1s, el riesgo de reca\u00edda sea mayor<\/strong>.<\/p>\n<p>En esta fase, y en todas las fases de la recuperaci\u00f3n, el<strong> papel del nutricionista<\/strong> es muy importante porque:<\/p>\n<p>&#8211; <strong>Encuentra un equilibrio entre mantener la masa muscular y evitar ganar grasas<\/strong> a trav\u00e9s de la alimentaci\u00f3n ajustado las calor\u00edas y los macronutrientes.<\/p>\n<p>&#8211; <strong>Controla la inflamaci\u00f3n sin bloquearla<\/strong> (de forma natural).<\/p>\n<p>&#8211; <strong>Aporta micronutrientes a trav\u00e9s de los alimentos y los suplementos<\/strong> (si fuesen necesarios) que ayuden en la cicatrizaci\u00f3n y recuperaci\u00f3n.<\/p>\n<p>&#8211; <strong>Prepara al paciente para volver m\u00e1s fuerte al campo<\/strong>.<\/p>\n<p>1. <span style=\"text-decoration: underline;\"><strong>C\u00f3mo calcular las calor\u00edas totales (GET)<\/strong><\/span><\/p>\n<p>Utiliza la <strong>f\u00f3rmula de Mifflin-St. Jeor<\/strong> para calcular el <strong>metabolismo basal<\/strong> (la energ\u00eda que gasto el cuerpo para sus funciones vitales). Al aplicarla, el metabolismo basal (MB) de Carlos es de <strong>1775 kcal<\/strong>.<\/p>\n<p>Para calcular el gasto energ\u00e9tico total (GET), hay que <strong>multiplicar el MB por<\/strong> el <strong>factor de actividad<\/strong> (1,25 porque apenas entrena) <strong>y<\/strong> por el <strong>factor de inflamaci\u00f3n<\/strong> (1,2). El resultado final GET de Carlos es de <strong>2662 kcal diarias<\/strong>.<\/p>\n<p>2. <span style=\"text-decoration: underline;\"><strong>C\u00f3mo ajustar los macronutrientes<\/strong><\/span><\/p>\n<p>&#8211;<span style=\"text-decoration: underline;\">Prote\u00ednas<\/span>: clave para no perder masa muscular, reparar el tejido da\u00f1ado y una correcta cicatrizaci\u00f3n. En este caso, la ingesta de Carlos cuando no estaba lesionado era de 1,6-2,2g\/kg\/d\u00eda, pero al estar lesionado, es ideal aumentar un poco m\u00e1s la ingesta. Por lo tanto, multiplicando la <strong>ingesta de prote\u00ednas (2,35g\/kg\/d\u00eda)<\/strong> por el peso (77kg), nos da un resultado de <strong>181g de prote\u00ednas al d\u00eda<\/strong>.<\/p>\n<p>&#8211; <span style=\"text-decoration: underline;\">Grasas<\/span>: clave para crear hormonas y controlar la inflamaci\u00f3n. La ingesta normal de Carlos cuando no estaba lesionado era de 1g\/kg\/d\u00eda, pero al estar lesionado, es ideal aumentar un poco la ingesta de grasas y crear m\u00e1s hormonas que ayuden en la recuperaci\u00f3n. Por lo tanto, multiplicando la <strong>ingesta de grasas (1,1g\/kg\/d\u00eda)<\/strong> por el peso (77kg), nos da un resultado de <strong>85g de grasas al d\u00eda<\/strong>.<\/p>\n<p>&#8211; <span style=\"text-decoration: underline;\">Carbohidratos<\/span>: fuente de energ\u00eda principal para evitar la p\u00e9rdida de masa muscular y soporte en el sistema inmune. Normalmente los carbohidratos <strong>se calculan restando las calor\u00edas totales (2662 kcal) menos las calor\u00edas de las prote\u00ednas (185g x 4kcal\/g = 740 kcal) y menos los calor\u00edas de las grasas (85g x 9kcal\/g = 765 kcal)<\/strong>. El resultado es de <strong>1157 kcal de carbohidratos<\/strong>. Para saber <strong>cu\u00e1ntos gramos<\/strong> de carbohidratos se necesitan, hay que <strong>dividir 1157 kcal entre 4 kcal<\/strong> y nos da <strong>289g de carbohidratos<\/strong>.<\/p>\n<p>El <strong>resultado de ajustar las calor\u00edas y los macronutrientes<\/strong> es <strong>generar un plan nutricional equilibrado y en el que el balance energ\u00e9tico sea 0<\/strong>.
